Mandatory Paperwork for ALL Athletes: 

In an ongoing effort to ensure the safety of our student athletes, next year the MSAA will require  a proof of physical,  along with proof of insurance.  Students WILL NOT be able to participate if they do not provide proof of both.

Mandatory Student Physical Form (must be completed by a physician)

Parent Permission-Insurance 19-20  (must provide copy of insurance card)

Release of Liability Form

Athletics Contract Form
